CareLinx by Sharecare is looking for a Clinical Manager to assist
with CareLinx's Health Partnerships on the Customer Success team.
CareLinx is a healthcare technology platform that connects families
with non-medical, in-home caregivers. This role will be primarily
supporting a large health plan whose members enroll in a
non-medical home care benefit through CareLinx as well as other
partnerships as needed. The Clinical Manager will be responsible
for monitoring and analyzing incoming data from tasks being
completed in the home of members and ensuring the data is accurate
and complete. Additionally, this role will help support members via
educational outreach and occasional outreach to members' primary
care providers to help coordinate care.
As a Clinical Manager, you are the guiding expert on using data to
identify clinical risks in the home for the health plan, or other
partner types, to take early intervention steps. You need to have
excellent communication skills and be comfortable working in
ambiguity on occasion. You should also have experience in data
analysis and be able to review data and identify gaps or potential
interventions to impact positive health changes. There will be
opportunities to affect program development through your clinical
experience and to make recommendations to health partners and the
rest of the CareLinx team from a clinical perspective. This may
include consulting with our partners to help strategize on
leveraging the CareLinx technology to help meet goals and
objectives for the program. You should be a team player and be
willing to learn about CareLinx's partners, to be a strategic,
clinical advisor. You need to thrive in a fast-paced,
technology-forward environment and be comfortable working from

Job Summary:
Manage a caseload of families who will be enrolling in a
non-medical home care benefit
Monitor incoming care plan task data, which caregivers complete
with members in the home and serve as a quality control to ensure
the proper tasks are associated with the correct members
Liaise with internal CareLinx teams to ensure caregivers are
completing care plan tasks as intended
Analyze and identify in-home care risks from data and escalate to
the health plan as appropriate
Work collaboratively and professionally with other team members and
teams within CareLinx (e.g., Product, Customer Success, Operations,
Engineering, etc.)
Document accurate and complete notes in CareLinx's system related
to any interactions
Participate in meetings with health partners' clinical team members
and report out on findings and best practices
Outreach directly to members to follow up to gather additional
information after health status changes like recent falls, chronic
illness, etc.
Assist with coordinating referrals to additional social and
clinical benefits and services within health partner resources
Create composite reports for partners and internal executive
management using incoming data to track key metrics, program and
partner progress
Additional job duties may be assigned on an as-needed basis

Experience working with Microsoft Office/Google Suite

judgment
Active license as a US-based, registered nurse (RN)
Preferred Qualifications:
At least some clinical quality work experience
Familiarity with in-home care
Experience developing individualized and program-specific care
plans
Bachelor's Degree
