BON SECOURS HOME CARE, LLC. POSITION SUMMARY Under the direction of the Director, responsible for the implementation, delivery, coordination and supervision of hospice services delivered by respective team members consistent with Bon Secours Hospice policies and procedures. Coordinates team activities including assignment of primary cases, case conferencing, lab reporting, admit and discharge tracking, documentation review, and monitoring of timely documentation submission. Encourages appropriate utilization and goals to meet the patient's needs. Oversees field orientation of new staff members. JOB REQUIREMENTS 1. Possession of current license to practice as registered professional nurse in the state of Virginia. 2. Work requires a minimum of three (3) years health care related experience. One to two years in home health or hospice required. One (1) year of supervisory experience and/or Hospice House experience preferred. 3. Work requires that the incumbent maintain CPR certification. 4. Work requires a high level of knowledge and proficiency in Hospice nursing practice, techniques and methods, and Medicare/ Medicaid/JCAHO knowledge. 5. Work requires previous work experience using a variety of diagnostic and treatment related products, tools and manual and electric devices (e.g., IV pumps, blood collection devices, mechanical lifts, wound care products, etc.).6. Possesses valid Virginia driver's license or provides own transportation to/from patient's homes and the office.7. Demonstrated skill in administrative performance, including expertise in both written and oral communication, supervision of staff, documentation requirements, service coordination and case management.8. Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management and hospice nursing.
