Provides basic therapeutic nursing care according to established standards of practice under the direction and supervision of the RN. Provides care to those patients whose care requirements are routine and standardized with predictable outcomes. Assists the RN and healthcare team to achieve desired patient outcomes. Communicates effectively and works cooperatively with others. Has respect for and understanding of other clinical disciplines. Demonstrates knowledge of quality improvement initiatives and participates in performance improvement activities. Assumes responsibility for self-development in life-long learning. Maintains competencies within practical nursing standards and seeks opportunities to develop professionally.

A Typical Day As An LPN Consists Of
Triaging Patients
Checking Vitals
Rooming Patients
Documentation in EMR (EPIC)
Prescription Refills
In Basket Management
Patient Calls and Education
Vaccines and Injections
Assisting with Procedures

What You Need

Education: Graduate of an accredited LPN school

Certifications: BLS Certification (from the American Heart Association)

Licensure: Eligible for licensing in the commonwealth of Virginia as a LPN

Skills: Proficient in English, verbal and written communication skills, and computer skills required

Job Responsibilities Provides direct patient care utilizing the nursing process under the direction and supervision of the RN.
Reviews diagnostic test results and reports abnormal results and changes in patient condition to the RN, physician, and appropriate healthcare team members in a timely manner.
Collaborates with the supervising RN on the execution of the patients plan of care.
Administers prescribed medications, therapies, and treatments in accordance with practical nursing standards.
Maintains and reviews patient medical records, notes and physician orders; documents all patient care activities in accordance with documentation standards.
Assists physicians, RNs and health care team members with data collection, assessment, planning, intervention, and evaluation of patient and families care to achieve desired outcomes.
Implements components of the plan of care within the LPN scope of practice and competency to promote desired outcomes.
Assists with discharge planning and carrying out the patients teaching plan under the direction of the RN.
Evaluates and maintains the environment to promote patient and healthcare team safety.
Collaborates with the RN and healthcare team to advocate for the patient and supports the patients psychological, ethical, spiritual, and cultural needs.
Provides compassionate basic nursing care; maintains patient privacy and confidentiality.
Effectively communicates with health care team members, patients and families; supports healthcare team members to achieve team goals. Assigns appropriate tasks to care assistants.
Seeks out healthcare team members to serve as a resource to increase practice knowledge.
Keeps informed of unit initiatives, supports the outcomes of the team/ committees work into practice.
Participates on unit teams, work groups and/ or committees. Demonstrates willingness to give 100% and voluntarily assists others.
Works with the team to achieve maximum productivity.
Demonstrates knowledge of unit based quality improvement initiatives.
Participates in performance improvement initiatives.
Identifies own learning needs based on self-evaluation and the progress made from the previous years goals.
Assists with the orientation of new employees under the supervision of the RN.
Seeks opportunities to further increase practice knowledge.
Maintains own Employee continuing Education Record and outlined core and specialty competencies relevant to area of practice.
