Community Health Coordinator-- Licensed Practical Nurse -Note:
This job requires regular travel within the Kilmarnock,
Tappahannock, West Point and Gloucester areas. Travel is reimbursed
at the IRS Standard Mileage Reimbursement Rate.Master Center for
Addiction Medicine is a physician-led addiction treatment program
for patients overcoming alcohol or drug addiction with care built
around the concept of medication-assisted treatment (MAT). We are
headquartered in Richmond, Virginia with additional locations in
other areas of Virginia. -We are an outpatient clinic with no
overnight hours. We do have evening and weekend hours.We seek a
skilled, empathetic Licensed Practical Nurse passionate about
leading, making a difference, and improving the quality of life for
individuals with substance abuse addiction. We are looking for an
LPN to serve as traveling medical support -within the Kilmarnock,
Tappahannock, West Point and Gloucester areas. -Licensed Practical
Nurse (LPN) certification required.A comprehensive benefits package

bottom of job ad for more information.Position Summary: The RN/LPN
Community Health Coordinator will be a key member of Master
Center's treatment team that includes providers, therapists,
medical assistants, and peer recovery coaches who collectively
strive to provide quality care to patients on the road to recovery.
This position requires close collaboration with addiction medicine
providers, peer recovery coaches and other medical support staff. -
- -for patient care and EMR documentation.Hours: Full time (40
hours per week). This position will primarily rotate between
Kilmarnock, Tappahannock, and Warsaw. Travel to other Master Center
offices in Richmond, Chesterfield, Gloucester, and Hampton may be
required as needed. Some evenings and weekends may be
required.Duties and Responsibilities: -
Maintains accurate knowledge of
substance use disorder trends, its effects on the body and
cognitive functioning, signs and symptoms of narcotic overdose, and
the overall implications of the disease of addiction. Collaborates
assertively as part of an interdisciplinary and interdepartmental
team to provide, observe, communicate, and encourage interventions,
treatments, and behaviors that promote holistic well-being.Creates
healthy interpersonal interactions with patients.Performs new
patient intakes, reviews medical history and medication changes,
documents vital signs and breathalyzer for all patient visits.
-Communicates patient vitals and health status to medical providers
that are treating via telehealth.Coordinates with peer recovery
coaches assigned to areas of coverage.Performs urine drug screens,
pregnancy tests as needed and draws blood. Processes urine and
blood samples according to guidelines. Obtains physician orders;
orders medications; collaborates with pharmacy; transcribes orders.
Assists with acquiring pre-authorizations of medication refills per
pharmacy/insurance request Maintains thorough documentation of
daily activities including nursing, treatment services, crisis
intervention and case management. Oversees all individuals
receiving medical services, administers medication, guides, and
responds to inquiries regarding patient health issues.Acts as
liaison, advocate, and coordinator for patients receiving services,
collaborating with Master Center's Healthcare Navigators when
needed.Implements and follows up on all support service
recommendations. In consultation with the pharmacist, provides
education to patients about potential drug interactions.
Works with the Medical Operations
Manager to complete and maintain requirements of the alternate
delivery license with the Board of Pharmacy. Responsible for
receiving and securing all medications on premises. Responsible for
monitoring requests for medical records and ensuring compliance
with 42 CFR and HIPAA standards. Complies with all rules,
regulations, and policies of the organization.Other projects and
duties as assigned.Knowledge, Skills and Abilities:
Excellent communication and
interpersonal skills.Exceptional prioritizing, time management and
organizational skills.Strong written and analytical
ability.Self-motivated, proactive, and must have the ability to
work independently and in a collaborative team environment.Must
demonstrate strong problem-solving skills, high attention to detail
and an aptitude for learning.Knowledge of regulatory standards and
compliance requirements.Working knowledge of EMR computer and
software applications used in job functions. (Experience with
E-Clinical Works a PLUS!).Requirements:
Require License or eligibility to
practice as Registered Nurse/Licensed Practical Nurse by Virginia
State Board of Nursing in the State of Virginia.Require graduation
from an accredited school of registered nursing. BSN
preferred.Strongly prefer experience working with people with
mental illness, geriatric issues, and/or substance abuse.Benefits
