Excellent Salary, based on experienceGreat benefit packageJob Description:To manage and coordinate the care of assigned clients in their homes by providing comprehensive skilled nursing services as needed to achieve maximum rehabilitation and recovery for the clientResponsibilities Include:Completes Initial comprehensive assessment of clients needs in order to achieve determined goals Accept client admissions as assigned by agency Provide all required skilled services as ordered by the physician Observe signs and symptoms and report to the manager as needed to achieve patient goals Oversight of collective care team to achieve common goals for the patient Promotes client and caregiver independence in self care Maintains comprehensive administrative, and medication records Consults with physician when necessary to achieve patient goals
