We are looking for a seasoned Family Nurse Practitioner to care for Pediatrics and Adult Care, who is also comfortable providing some basic prenatal care. Some highlights of this position include:
* Locations: Halifax, Harrisonburg, and Richmond, Virginia
* These clinics will provide primary, prenatal, and postnatal care in an outpatient setting with the exclusion of delivery.
* Minimum of 3 years' experience as a licensed provider treating family medicine, pediatrics, and adults with complex issues
* Full-time position with non-traditional hours of noon to 6:00pm or 7:00pm, plus
1-2 Saturdays per month
* Shared call schedule, with RN first call
* Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients' health holistically.
* Review quality and outcome metrics monthly with the healthcare team
* Collaborate with local civic and faith-based leaders on community education programs
* Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice

Requirements for this position include Graduate from an accredited School for Nurse Practitioners, and current and unrestricted Virginia license with Family Nurse Practitioner specialization and DEA. Candidates must have the ability to provide quality, comprehensive, continuity of medical care in a compassionate manner with minimum supervision.
The Nurse Practitioner (NP) is responsible for the provision of health care services to individuals, families, and/or groups who receive their care under the auspices of Sentara. Health care services include conducting health assessments, prescribing/providing treatments/interventions, promoting health, and providing disease prevention and management. Health care services are provided under the direction and supervision of assigned primary care physician (collaborative agreement). The Nurse Practitioner is expected to participate in quality assurance and improvement activities, engage in member of the team activities, and use resources (human and material) appropriately.
Nurse Practitioner experience preferred.
