Job Title: Home Hospice Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (10:00 - 18:00) Experience: Minimum 1 year in home hospice case management License: Registered Nurse License required Certifications: BLS required Must-Have: Experience in home hospice case management Description: The role involves managing patient care in a home hospice setting. Candidates must have experience with home hospice case management. The average census for this position is approximately 35-45 patients. BLS certification is required for this role. Candidates should be prepared for possible floating based on facility needs. All applicants must have a valid registered nurse license. Epic charting experience is mandatory for this position. NIH and stroke education is required for RNs in this specialty. Specialty experience within the last year is required for all applicants.
