We are dedicated to delivering exceptional, personalized care that enhances health, reduces costs, and empowers individuals to lead fulfilling lives. With a strong foundation in clinical assessment and care management, our nationwide network of clinicians transcends traditional barriers to healthcare by reaching members in their own homes. We assist older adults and other at-risk populations in achieving better care quality, improving health outcomes, and identifying chronic conditions that might otherwise remain undetected. Our job openings enable you to utilize your skills and compassion to make a meaningful difference in the health and well-being of others. Join our team and enjoy competitive pay and flexible scheduling while positively impacting the community! Responsibilities About the Role During visits that may last up to one hour, our providers assess and observe a member’s current health status, medical history, medication adherence, social circumstances, and other potential risks. This approach offers invaluable insights into a member’s overall health and well-being that are often missed during standard office visits. The Matrix Comprehensive Health Assessment (CHA) aims to enhance care quality and allows us to address multiple care gaps in a single visit. Our providers are responsible for: Conducting assessments for adults and geriatrics, including medical history, diagnosis and treatment, health education, referrals to physicians and case management, follow-ups, and thorough documentation in line with our guidelines. Administering and ordering necessary point-of-care and lab-based tests. Collaborating with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as needed. Working alongside Primary Care Physicians (PCPs) to provide patient education and follow up as required. Delivering services at the member’s residence, whether in their homes or Skilled Nursing Facilities. Benefits of Working with Us Flexibility – Schedule visits according to YOUR availability. Assessments are conducted from Sunday to Saturday between 8 am and 7 pm, allowing you to work during the day, evening, or weekend based on your preferences. Competitive Pay – Your earnings are based on the number of assessments completed. You can increase your income by being more available and traveling to see members outside your home area. Those averaging 20 to 100 visits monthly can earn between $1,800 and $11,500 per month, depending on the per assessment rate of $90 to $115 based on your state. Along with comprehensive onboarding training, you will be assigned a preceptor, receive personalized feedback, and gain free access to continuing education opportunities.
