We are an office environment, provide--telephonic patient care--and
work with practice staff so there are no beds in the office.
Position Summary: Nurse who will provide assistance to providers
within the VCP network. This support will include, but not limited
to, monitoring of progress towards network goals, gap management,
practice transformation to improve process flows, transitions of
care for identified populations, and care coordination to drive
quality and cost outcomes for the attributed populations.
- Assess assigned practice workflows and processes that are in
place to close gaps in care, make outreach to patients for
transitions of care, and the ability to perform care coordination
activities and then assist with improving those processes to
achieve the desired results within the network.--
- Identify attributed patients who benefit from care coordination
and work with practice to effectively manage the high risk, high
cost, and high utilizers.
- Assist with referring to appropriate ancillary services such as
payer specific CM/DM programs, BH resources, pharmacist support,
and other resources as identified.
- Assist assigned practices with managing their populations that
can include, but not limited to, specific chronic diseases, high ER
utilizers, medication compliance, and other identified areas of

practice with network and develop a plan to work towards the
identified network goals.
Qualifications --Education and Experience:
- Licensed Practical Nurse Required.
- Case management experience preferred.
- Knowledge of primary care practice processpreferred
- Experience with evaluating and monitoring dataaround quality
--Knowledge, Skills, & Abilities
- Proficiency with MS Word, Excel, and Power Point required.
Experience with SharePoint preferred.
