Great work you’ll do here:

The Hospice Care Coordinator is responsible for compiling referrals from referral sources in a timely, efficient manner while maintaining positive working relationship with all internal and external customers. This position will support hospital(s), clinics and nursing facilities. It will be important to assure quality customer service to clients, families, referral sources and co-workers.  This role will also serve as the Hospice Consultant, educator, and customer services specialist to referral sources. The Hospice Care Coordinator will assist in meeting agency monthly targeted Medicare admission goals.

  • Maintain confidentiality of information relating to the patient and family
  • Evaluate patients for Hospice appropriateness and defines disciplines appropriate to meet client’s needs in the Hospice setting
  • Facilitate daily visits for patients in the GIP Level of Care
  • Communicates effectively and positively with clients, families and both internal and external staff
  • Begins the admission process by accepting referral, obtaining initial intake information and accepting physician orders
  • Validates payer information/insurance benefit coverage for Hospice
  • Resolve issues and concerns between the referral source, the company and the client and/or families
  • Educate healthcare professionals and clients on Hospice services and acts as a resource to referral sources, clients and staff concerning eligibility for Hospice services
  • Maintain a weekly call plan, document referrals processed on a weekly basis and referral source contact on a daily basis
  • Ensure referrals meet regulatory criteria
  • Collaborate with service providers to identify appropriate Hospice patients, inclusive of maintaining a cohesive relationship w/ Home Health to provide continuum of care
  • Maintains open communication with office, Account Executive and Administrator on growth with assigned accounts
  • Facilitate referral by fax or phone to office
  • Compile necessary paperwork as needed with referral source for the agency to start the patient’s services
  • Maintain monthly call back program for the agency
  • Complete other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

  • Bachelor’s Degree in Nurse is preferred
