Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for adults with severe mental illness (SMI) who are most at-risk of psychiatric crisis, hospitalization, and involvement with the criminal justice system. ACT is one of the oldest and most widely researched evidence-based practices in behavioral healthcare for people with severe mental illness.
ACT uses a multidisciplinary team approach with assertive outreach in the community. The team is comprised of a Supervisor, Team Lead, SUD Clinician, Vocational Clinician, Case Managers, Nurses, Peer Support Specialist and Administrative Assistant. The consistent, caring, person-centered relationships have a positive effect on outcomes and quality of life for the individuals that we serve.
